You'll want to create A records in both your internal and external DNS. For server 2008, it's under DNS manager, Forward lookup zones. Create the A record in the zone that matches your external domain name. Depending on who manages your external DNS the steps might be a little different. Same basic idea applies, of creating the name you want in your forward lookup zone with the needed IP address.
For example if your company is Contoso and you want the FQDN to be kpi.contoso.com, you'll put the "kpi" A record in contoso.com forward lookup zone on your internal DNS server pointing to the internal address. For the external record, create an A record named "kpi" with the external IP that NATs to your internal IP address hosing the Visual KPI Server.
